QUAD BLASTER AGREEMENT

I acknowledge that I am aware that certain risks and dangers are inherent in any exercise undertaking and therefore, in the use of the Quad Blaster.  These risks include, but are not limited to, the possibility of injury whether or not the Quad Blaster is used properly and according to instructions.  I agree to assume responsibility for all risks by using The Quad Blaster and waive, release and discharge Carlos DeJesus and Personal Fitness Concepts from any and all claims for damages, for death, personal injury, or property damage which I may have as a result of my use of the Quad Blaster.
